NUSAC uses several forms for it's day to day operations, these can all be downloaded here. Forms marked with an asterisk can also be downloaded partially completed from the database.

Membership Forms

• *BSAC Membership Form - Required to join NUSAC. (you will need three copies, one for us, one for you and one for BSAC.)
• BSAC Direct Debit Form - Only required if wishing to pay BSAC by Direct Debit.
• NUSAC Membership Form - Only required if your database record is blank.
• *Self-Declaration Medical Form - Required to dive with NUSAC, you and the club must have a copy which must be renewed annually.
• *SU Associate Membership Form - Required for non-students wishing to join NUSAC.
